In accordance with PADI and the Recreational Scuba Training Council guidelines, the following forms must be completed and presented to Chicagoland Scuba prior to beginning any training, at any level. Please take a moment to complete these forms prior to the beginning of your class.

Medical Statement Form	This form lists several contradictions to Scuba Diving as determined by the Recreational Scuba Training Council and PADI. Please review this document carefully. A positive response to any of the conditions listed on the form does not necessarily disqualify you from diving. A positive response means that there is a preexisting condition that may affect while diving, and you need consult with your physician prior to engaging in dive activities.
Liability Release and Assumption of Risk Form	This form ensures that you understand that there are certain risks inherent to skin and scuba diving, and you accept these risks. Please read this form carefully and, if you agree, sign at the bottom. Your instructor will assist in filling out some of the blanks, and answer any questions you may have before signing.
Standard Safe Diving Practices Form	This form makes sure you are informed that there are established safe diving practices. Please read the form and, if you agree, print your name at the top, and sign at the bottom. Your instructor can answer any questions you may have prior to signing the form.
Student Contract	All students are required to read and sign this contract at time of registration for all courses. This explains what we expect our of our students, and what you should expect out of us as professional instructors.
